PUTNAM COUNTY, W.Va. (WSAZ) - One person has died as a result of a two-vehicle crash Tuesday morning on Interstate 64, the Teays Valley Fire Chief said.

The crash, which involved a car and a dump truck, was reported just before 11 a.m. near the Bills Creek overpass, about a mile from the Winfield exit. Investigators say the car rear-ended the dump truck.

The passenger died at the scene.

The driver was taken to St. Mary’s Medical Center in Huntington with serious injuries. Both victims were trapped in the wreckage and had to be extricated.

The westbound lanes have since reopened in that area.

The accident happened near the Bills Creek overpass, about a mile from the Winfield exit.

Two people were trapped in the wreckage, but further details are unavailable at this time.

PUTNAM COUNTY, W.Va. (WSAZ) - A crash involving a dump truck and a car has closed the WB lanes of I-64.

The Hurricane Fire Department said the crash happened right near the Bills Creek overpass about a mile from the Winfield exit.

Crews on scene tell WSAZ that a car crashed into a dump truck, and two people are trapped as a result of that crash.

All Westbound lanes are closed right now. Use caution if traveling through the area.

Woman dies in car crash on I-64 in Norfolk

NORFOLK — Virginia State Police are investigating a fatal two-vehicle crash that occurred early Wednesday morning in Norfolk.

Around 2:44 a.m., officers responded to the westbound lanes of I-64, west of Granby Street, according to a state police spokesperson.

The two vehicle fatal crash occurred inside the safety work zone area.

Shortly before officers arrived on scene, the male driver of a 2006 Ford Fusion traveling on I-64, made a sudden lane change, ran off the right side of the roadway, and struck the jersey wall.

The male driver then overcorrected back into the left lane, which was closed off, and struck the rear of a parked Sweeper Truck, coming to rest underneath the truck.

The driver of the sweeper truck was uninjured. The male driver of the Ford Fusion was taken to the hospital with life-threatening injuries and the female passenger died on impact.

Police later identified the the victim as Samantha Leigh Sims, 21, from Portsmouth.

The westbound lanes of I-64 were shut down for about five hours during the course of the investigation and for the removal of the vehicles, according to a press release from the state police.

The Virginia State Police Crash Team was called upon to assist in the investigation. Police are working to identify and notify the victim’s family.

The crash continues to be under investigation.

Passenger dead, driver seriously hurt after crashing into sweeper truck in work zone on I-64 in Norfolk

NORFOLK, Va. (WAVY) — A female passenger died and a male driver was seriously hurt after a crash in a work zone on I-64 in Norfolk on Wednesday morning.

The crash happened around 2:44 a.m. in the westbound lanes of I-64, west of Granby Street, Virginia State Police say.

Police say there were multiple work trucks with lights and another proper signage warning of the work zone at the time of the crash when the driver of a Ford Fusion was traveling at high speeds. Police say the male driver made a sudden lane change, ran off the right side of the road and struck a Jersey wall before overcorrecting and striking a parked International sweeper truck in the work zone.

Police say the Fusion came to rest underneath the truck and the female passenger died on impact. The driver was taken to Sentara Norfolk General with life-threatening injuries. The driver of the sweeper truck wasn’t hurt.

All westbound lanes of I-64 were shut down for about five hours for the investigation and removal of the vehicles. The crash remains under investigation.

Police have identified the deceased female as 21-year-old Samantha Leigh Sims of Portsmouth.

Troopers: 2 killed after SUV rear-ends dump truck on I-64

LOUISA COUNTY, Va. -- Two people were killed a in wreck involving a dump truck along Interstate 64 in Louisa County Tuesday night, according to officials with Virginia State Police.

Troopers were dispatched to the wreck on I-64 east near Route 15 (mile marker 133) just before 11 p.m.

"The preliminary investigation reveals a Honda SUV was heading east when it struck the rear of a 1990 GMC Dump truck," Sgt. Dylan Davenport with Virginia State Police said.

Two adults in the SUV, who were both wearing their seat belts, died of their injuries at the scene, Davenport said.

The dump truck driver, a 51-year-old Fluvanna County man, was transported to an area hospital with injuries that were not life-threatening.

There was no word if he was wearing a seat belt.

Police said the names of the victims will be released once their next of kin have been notified.

Davenport said the investigation into the wreck is ongoing.

Father and his 3 children killed in horrific I-64 crash in New Kent, driver may have suffered medical emergency

RICHMOND, Va. (WRIC) — The identities of the people involved in a fatal crash on Interstate 64 in New Kent County on Monday have been released. The family of five was vacationing in Virginia Beach heading back to home to Kentucky when tragedy struck.

Anthony Snow, 53, of Covington, Ky. and his three children died at the scene of the fiery crash that shut down a portion of I-64 for hours. The accident happened at exit 214, the Providence Forge exit.

Joseph Snow, 13, Mary Snow, 11, and Victor Snow, 9, were all passengers in the back seat of a 1995 BMW sedan and lost their lives at the scene.

The children’s mother was the front-seat passenger in the vehicle, 34-year-old Ishia Palmisano of Newport, Ky. She was rescued from the wreckage and taken to VCU Medical Center for treatment of serious injuries. A close friend says Palmisano and Snow were in a long-time relationship and their children were their lives.

Family says she underwent surgery for several shattered bones and injuries sustained in the wreck. Family members also tell 8News she has a long road to recovery ahead of her, but are thankful she is fighting and live amid the painful tragedy.

The accident occurred just after 11 a.m. July 12. The BMW ended up traveling along the right shoulder before crashing into a Virginia Department of Transportation traffic cam pole, catching fire. Both lanes of the interstate were closed for a time, and traffic was backed up as far as eight-and-a-half miles while crews worked the scene.

Virginia State Police are investigating a medical emergency as a possible contributing factor into the cause of the crash.

Those close to the family say Palmisano is alert and has been cooperating with police, saying that she was asleep in the passenger seat and woke up when she felt the vehicle shift directions. She looked over and saw Snow slumped over the steering wheel. Palmisano then attempted to grab the steering wheel before it crashed, but it was too late.
